The Fylde Coast was selected as one of the first ‘vanguard’ sites in England to test new ways of organising and delivering health and care services which, in future, could potentially be rolled out across other areas of the country.
NHS Blackpool CCG, NHS Fylde and Wyre CCG, Blackpool Teaching Hospitals NHS Foundation Trust, Lancashire Care NHS Foundation Trust, Blackpool Council and Lancashire County Council – are working together to implement these plans, called ‘new models of care’.
These new models of care have been developed to address challenges the health and care system on the Fylde Coast faces and make sure that the services provided are better coordinated by taking into account the health, social and emotional needs of residents.
The new models of care being implemented are extensive care and enhanced primary care. Together, these care models will provide one integrated health and care system which is able to provide seamless, proactive, out of hospital services by focuses on meeting the full spectrum of an individual’s health and wellbeing needs. This will radically transform the outcomes and experiences of residents.
