Better Care Together is made up of 11 organisations including NHS Trusts, Local Authorities and Clinical Commissioning Groups (CCGs) who are working together to provide improved models of care for patients across North Lancashire and South Cumbria.
Their vision is to secure safe, high quality health services in partnership with professionals and patients and to give local people the best opportunity to live longer and healthier lives. They aim to manage competing demands by ensuring that their priorities are based on the needs of the population and the highest possible quality standards and health gain that they can commission, for their population, within their available resources.
The Better Care Together vision is: “Our vision is to see a network of communities within the Morecambe Bay area enjoying great physical, mental, and emotional wellbeing, supported by a health and care system providing great care that is recognised as being as good as it gets”.
The vanguard teams put communities and patients at the heart of everything that they do and, where appropriate and within their resources, provide care close to where people live. They aim to influence the health inequalities that exist within the population. The Test Bed programme will be working with five of the 12 Integrated Health Communities (ICCs) located within North Lancashire to bring the pilot to residents in this area.
