Better Care Together is made up of 11 organisations including Trusts, Local Authorities and Morecambe bay Clinical Commissioning Group (CCG) who are working together to provide improved models of care for patients across North Lancashire and South Cumbria. Integrated care communities (ICCs) are a sub group of the Better Care Together Vanguard.

They will see health and social care professionals, GPs, the voluntary sector and the community working as one team within one system to support the health and care needs of population it serves. They will focus on helping the population to manage long term health conditions and improve access to information about healthier lifestyles locally.

Evidence shows that the most successful ICCs will reduce the overall number of people who need to be cared for in hospital and improve the health and wellbeing of communities. The Better Care Together Test Bed is being delivered initially to Bay ICC and will be rolled out to both Carnforth and Lancaster ICC in the future.

As part of Better Care Together we want to deliver a better future for health and wellbeing by:

  • Enabling our communities to be as healthy as possible.
  • Making sure that health and care services are provided across North Lancashire and South Cumbria are the best possible.
  • Meeting the needs of the local population, now and well into the future.
  • Making sure women’s, men’s, mental health, children’s, and older people’s services receiving equal priority with all other areas of care.

How we plan to do this:

  • Health professionals will work in local sites sharing their expertise with GPs and community teams.
  • They will also work in partnership with people to keep people fit and well.
  • They will help people manage their own condition—this could be at home or in their local community.
  • Developing an ‘Accountable Care System’, to allow organisations to work more closely together.

The vanguard teams put communities and patients at the heart of everything that they do and, where appropriate and within their resources, provide care close to where people live. They aim to influence the health inequalities that exist within the population.
